Weekly assignment record sheet/rubric?

    I’m looking for a new type of weekly assignment sheet that will allow me to record my evaluations of my children’s narrations and work for each subject each day that I meet with them.  I want something that will allow me to record the time spent (for high school credit hours) on the subject as well as a space to write a few notes on how they did with the assignment that day.

    I’m finding it difficult to always set aside time at the end of each week to make sure I’m recording my thoughts for all subjects for all students (I have a LOT of students at my table right now, my head is spinning) and I’m falling behind on keeping up with recording the time worked on subjects for my high schoolers.

    Also, if anyone has seen or made a sheet that includes a rubric of some kind that helps me rate their narrations/efforts for group and independent work, that would be lovely.

    I know I could make this myself, but I sure don’t want to put all that time into it if there is something out there similar to what I want that I could use or tweak for my purposes.

    Any ideas or thoughts or resources for me?

    I don’t think it has a rubric, but this portfolio pack from Not Consumed has a really nice weekly checklist with a place for grades/feedback.
